                     1) horse's PINK CTHS certificate
                     2) Certificate of Service if  horse not yet  registered
                     3) copy (front & back ) of certificate if from other registry
                     4) Bill of Sale or Transfer form if applicable

Fees will be collected at the site (in ADVANCE if Central Inspection):

 $50.00 per site (if more than 5 horses to be inspected no fee)
 $25.00 branding fee. Inspection  fee  $65 - $200 (depends on status/age of horse, see Fee Schedule) $50.00 Late filing fee may be charged.
